Continuity of mass boundary condition between different porous layers

Hi,
I am modeling heat and mass transfer in human artery wall (considering artery wall as multilayer). I need to know how to apply continuity of mass between different layers, incorporating Staverman reflection effects which is:

[(1-sigma).v.c-D.grad(c)]+ = [(1-sigma).v.c-D.grad(c)]-
where,
sigma = Staverman reflection coefficient
v = filtration velocity
c = concentration
D = diffusion coefficient
