holes in the mesh!

hello,

i have a problem i can solve.
i making a simple EM model in 2D.
for some reason when i making the mesh smaller , holes are starting to emerge.

it is like the triangles from the start of the model (x=0) are connecting to somewhere in the model space.
because i cant create a good mesh i can not solve the problem.
anyone maybe know what is the problem and how to solve it?
